St Monica’s Parish rejoices and celebrates the birth of every child.
This gift of life and love to every family is celebrated and warmly welcomed into our Parish family in and through the Sacrament of Baptism.
 
Please find the Application Form below.  Available dates are on the "pull-down" (unfortunately, we have no available dates at the moment) .  Dates chosen are not guaranteed until confirmed by the Pastoral Assistant.  Once we receive the form, the Pastoral Assistant will be in contact with you.  If you live outside the parish boundary, please send a Letter of Permission from your Parish Priest to Abygaile.Cukavac@cam.org.au immediately after you send you form.  We will not be able to process your application unless we receive this letter so please attend to it immediately.

The stipend for St Monica’s is $200 (payable at time of booking) and covers the administration fees, the Church and its upkeep, the Priests and Office Staffs’ time in administering, the Baptism and all its rites. The fee is due at the time of booking the baptism and can be paid via credit card, eft, cash, money order or cheque.

Process of Baptism 

  1. Application Form sent.  Permission letter sent to Abygaile.Cukavac@cam.org.au if not from the parish boundary.
  2. Pastoral Assistant to confirm one of 2 dates chosen by email.
  3. Baptism date booked.
  4. Compulsory Baptism Information Evening held usually on the first Wednesday of the month prior to your baptism date at 7:30 pm. One parent need to attend.  Invitation also extended to at least one god-parent. This is in the Symons Centre, the building adjacent to the church.
  5. Rites 1 of Baptism on the first Sunday Mass chosen.
  6. Rites 2 of Baptism on the second Sunday Mass chosen.
  7. Baptism day at mid-day (weeks 1 & 3 of the month).

    Godparent Declaration

    There must be one Catholic Godparent who has received the Sacraments of Christian initiation; Baptism, Confirmation and Eucharist, and belongs in an active sense to the Catholic Church. There may be a second Godparent who may be a Catholic, meeting the conditions just mentioned, or a baptised non-Catholic who is an active member in his/her own Church, representing the Christian Community. I/We understand that our child is being baptised in my/our Catholic faith and we know that it is my/our responsibility to educate the child in the practice and knowledge of the faith.

Verification of Baptism

At times we receive requests for Baptism verifications (for people already baptised at St Monica's whose original certificate has been lost or misplaced).
